IN MEMORIAM NOTICE 2004
Dear
Member,
It
is customary to devote a section of our High Holy Day issue of the Bulletin to
remember the departed. In order to prepare our In Memoriam Section in time for the September edition we must have
our listings complete by August 18, 2004.
As
has been our practice, we will print
all In Memoriam notices that were listed last year, unless we receive requests for cancellation by August 18, 2004.
Of course, we must be informed of additional listings by that date as well.
If
you wish any new listings, additional listings or merely to confirm or correct
your previous ones, please return the form below with your check for $5.00 per NAME to be
memorialized. If we do not hear from you
by August 18, 2004, we will print your In Memoriam listing as they have
appeared previously and bill you accordingly. We ask that all information
be submitted in writing, not over the telephone.
